Health and Safety Reimbursement Request form

The Health and Safety Reimbursement Request form is available on Child Care Links’ website at www.childcarelinks.org.  Click on “Providers” then click on “Forms” and scroll down to “Other”.

Licensed Center Based staff, Licensed Family Home Child Care Providers, License-Exempt Child Care Providers and In-Home Providers in the cities of Livermore, Pleasanton, Dublin and Sunol may request reimbursement for CPR, First Aid and Preventative Health training. Training must have occurred between July 1, 2014 and June 30, 2015.

Reimbursement of health and safety training costs depends on available funding and will be made on a first come, first served basis until the funds are expended.  Child Care Centers may be limited to reimbursement of 5 employees based on available funding.  Each provider must pay $5 out of pocket before the reimbursement is calculated.  Reimbursement amounts will vary and will only cover a portion of the cost of the training.  Reimbursement will not exceed $100.

Make sure to take the reimbursement request with you to the training.  The trainer must complete a section of the form.  Incomplete and outdated versions of the form will be returned unprocessed. Please contact Donnamarie Fuller at (925) 249-3923 for more information.
